Texas Consumer Notice – Access to Health Records & Complaints
In accordance with Texas Health & Safety Code §181.105 (HB 4224), patients may use the following information to request records, contact the licensing authority, or file a complaint.
Requesting Your Health Records
Patients may request copies of their records by:
- Calling our office at 281-540-2225
- Emailing our office at info@newleafchiro.com
- Submitting a written request in person at the front desk
Please include the following information with your request:
- Full name
- Date of birth
- Phone number
- Specific records requested
Records requests will be processed in accordance with Texas law and HIPAA guidelines.
